The following details should be submitted by the local level to the Ministry through the Province and by the Province to the Ministry:-
(a) Name and number of the body corporate to which the permit has been granted,
(b) the area served by the organized bodies to which the license has been granted,
(c) The conditions prescribed for the duration of the license of the organized organizations to which the license has been granted,
(d) Number of projects conducted by the province or local level or other necessary details,
(e) Other details deemed appropriate by the state and local levels.
